29 CFR 1926.501(b)(14): Each employee working on, at, above, or near wall openings (including those with chutes attached) where the outside bottom edge of the wall opening is 6 feet or more above lower levels and the inside bottom edge of the wall opening. a. On or about April 10, 2017 employees walking from the stair tower to the balcony landings of the project were not protected by personal fall arrest systems, guardrails or safety nets at the 18 inch wide space between the stair tower vertical support and the corner of the concrete balcony column, at the balcony ledge, exposing employees to fall in excess of 20 feet to the ground below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On or about April 10, 2017 employees working on a multi floor, multi unit high rise construction project were not protected by the use of guardrail systems, safety net systems, or personal fall arrest systems at the exterior balconies exposing the employees to falls in excess of 30 feet to the ground below. b. On or about April 10, 2017 employees installing rough electric on the ceilings of the back balconies working from a baker scaffold were not protected by guardrails exposing the employees to falls in excess of 18 feet to the ground below. Although top and midrails were installed at the window openings, by the employees working on a 3 foot tall Baker scaffold they were working above the height of the top guardrail. c. a. On or about April 10, 2017 employees were walking up and down interior concrete stairs and were not protected by any means of guardrails on the stairs open side, exposing them to falls in excess of 12 feet to the ground below.
