29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:    On or about 04/11/2017 at the Heritage Church Re-Roofing Project, the employers and employees were using a ladder in the rear of the building to access the main roof and another ladder in the front of the building to access the front entrance roof and neither ladder extended 3 feet past the roof decks.

29 CFR 1926.1053(b)(21): Employees were not grasping the ladder with at least one hand when progressing up and/or down the ladder:    On or about 04/11/2017 at the Heritage Church Re-Roofing Project, the employer and the employees were climbing up the ladder to access the main roof and were using one hand to leapfrog from rung to rung while the other hand was holding a bundle of shingles.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not being protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about 04/11/2018 at the Heritage Church Re-Roofing Project, employees were installing shingles to the roof deck and exposed to fall hazards in excess of 17 ft, and there was no fall protection system in use.    Date by which violation must be abated:  Immediately upon receipt of citation.    Pursuant to 29 CFR 1903.19(d), the employer must provide  documentation that this violation has been abated within 10 calendar days of the date it receives this citation.
