29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end. On or about 4/12/2017 employees at the 295 Ridge Road roofing project were exposed to approximately 18' falls. Employees were engaged in roofing activities from a 24' aluminum platform that extended more than 18 inches beyond its support.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about 4/12/2017 employees at the 295 Ridge Road roofing project were exposed to approximately 18' falls. Employees were not provided a means of access to their ladder jack scaffold.

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On or about 4/11/2017 employees at the 295 Ridge Road roofing project were exposed to approximately 18' falls. Employees were engaged in roofing activities from a ladder jack scaffold without fall protection. Hercules Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.451(g)(1)(VI)), which was contained in OSHA inspection number 116077, citation number 01, item number 1b, and was affirmed as a final order on 03/23/2016, with respect to a workplace located at 195 Steele Road, Hartford, CT 06101.
