29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    (a) On or about 4/11/2017 - at Lot 2243 on Madison Place in the Riverbend at Old Cahaba Subdivision, Helena, AL, eye protection was not used when employees were operating a Hitachi NV 45AB2  1-3/4" Nail Coiler and a Rolair System Air Compressor to nail shingles exposing employees to flying particles.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a) On or about 4/11/2017 - at Lot 2243 on Madison Place in the Riverbend at Old Cahaba Subdivision, Helena, AL, employees were exposed to fall hazards up to approximately 10.2 feet above ground level while installing shingles.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    (a) On or about 4/11/2017 - at Lot 2243 on Madison Place in the Riverbend at Old Cahaba Subdivision, Helena, AL, 20 foot aluminum ladders used to access the roof were not extended at least 3-feet above the upper landing surface, secured to a rigid support, and/or a grasping device was not provided.
