29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section: a) Centre Square Commons, Building #1 - on or about April 12, 2017, an employee was observed painting from a Genie GS-2632 scissor lift with a working height of greater than 10 feet above grade without proper means of fall protection.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) Centre Square Commons, Building #1 - on or about April 12, 2017, employees performing gas line painting on a low-slope rooftop, approximately 24 feet above grade, were not protected by guardrail system, safety net system, or personal fall arrest system.
