29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     On or about April 12, 2017, at the above addressed site, employees were exposed to a fall hazard of approximately 9 feet 10 inches when performing roof work on a low slope roof of a residence while not being protected from falling to the lower level.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:     On or about April 12, 2017, at the above addressed site, employees were exposed to a fall hazard when gaining access to/and from a roof approximately 9 feet 10 inches in height while using an extension  ladder that did not extend at least 3 feet past the upper landing surface .

29 CFR 1910.1200(e)(1): Employer had not developed or implemented a written hazard communication program included the requirements outlined in 29 CFR 1910.1200(e)(1)(i) and (e)(1)(ii):     On or about April 12, 2017, at the above addressed site, employees were exposed to the hazards associated with chemicals including but not limited to Gasoline and Polyglass Modified Roofing Cement where the employer did not develop, implement or maintain a hazard communication program.
