29 CFR 1926.453(b)(2)(v): A full body harness was not worn and a lanyard not attached to the boom or basket when working from an aerial lift: a) On April 12, 2017, personal fall arrest equipment used by employees operating and working from a Genie Model S-45 LiftPro aerial lift was not attached to the designated anchor points on the basket of the lift, thereby exposing employees to the hazard of falling greater than twenty (20) feet.
