29 CFR 1926.20(b)(2): The employer did not ensure that such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  On or about April 4, 2017 and at times prior thereto, Raj Patel as the Controlling Employer did not ensure that:  a)  Employees of its subcontractor were allowed to work from a powered industrial truck with an unapproved work platform.  Employees were potentially exposed to falls of approximately 40 feet to the ground below; and  b)  Its employees were exposed to being struck by falling debris from an unapproved wooden box or being struck by by collapsing components of that unapproved wooden work platform.
