29 CFR 1910.147(c)(4)(i): Procedures shall be developed, documented and utilized for the control of potentially hazardous energy when employees are engaged in the activities covered by this section. a) On or about April 4, 2016, a maintenance employee suffered the amputation of the tip of their middle finger when it became caught between an in-going nip point while removing a belt from the large Boiler #2's fuel pump motor and fuel pump pulleys. The established Large Boiler #2 lock-out/tag-out procedures were not utilized prior to the removal of the fuel pump belt guard.

29 CFR 1910.147(c)(4)(ii)(B): Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; a) On or about April 4, 2016, a maintenance employee suffered the amputation of the tip of their middle finger when it became caught in an in-going nip point while removing a belt from the Large Boiler #2's fuel pump motor pulley and the fuel pump pulley. The employer's LO/TO specific procedures for the Large Boiler #2 failed to address the hazard of the residual or stored energy present in the turning motor, belt, and pulleys.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lock-out or tag-out procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1)-(6): a) On or about April 4, 2017, a maintenance employee suffered the amputation of the tip of their middle finger when it became caught between a belt and pulley while performing a monthly fuel oil test on the Large Boiler #2. The employer failed to implement energy control application steps in order to safely perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LO/TO device application [(d)(4)], dissipation of residual energy [(d)(5)(i), and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
