29 CFR 1910.147(c)(4)(ii)(C): The energy control procedure did not clearly and specifically outline the steps for placement, removal and transfer of lockout devices or tagout devices and the responsibility for them: a) On or about April 11, 2017, the lockout procedures for the Hunter 20 & 30 failed to clearly identify specific procedural steps for the placement of lockout devices. The machine specific procedure failed to instruct employees to apply a lock out device to the ball valve controlling the pneumatics when performing servicing or maintenance.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On March 22, 2017, the employer did not ensure the lifting arm associated with the weight lifter on the Hunter 30 carrousel was adequately guarded to prevent employees from being struck by the moving equipment.
