29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Processing Room - On or about April 13, 2017, a written hazardous energy program (lockout/tagout) consisting of specific procedures, employee training, and periodic inspections was not available for employees exposed to the hazards of inadvertent startup of equipment when performing service and/or maintenance on the dump tables, press machine, and other operating equipment.

The employer did not ensure that each powered industrial operator completed the required training and evaluation specified in paragraph (l). Employees were thereby exposed to struck-by and crush-by hazards.
