29 CFR 1926.502(b)(3): Guardrail systems were not capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied within 2 inches (5.1 cm) of the top edge, in any outward or downward direction, at any point along the top edge: VA Medical Center: Guardrail installed around the elevator shaft was not adequately anchored to prevent dislodgement and provide support for a force at least 200 pounds. MSK Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.502(b)(3), which was contained in OSHA inspection number 1137144, citation 1, item 1 and was affirmed as a final order on May 10, 2016, with respect to a workplace located at W.G. (Bill) Hefner VA Medical Center, Salisbury, NC, 28144.
