29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about April 12, 2017, employees were exposed to struck-by and amputation hazards when energy control procedures were not utilized in instances including, but not limited to the following:    a. Greenerd press 512  procedures not utilized to prevent accidental activation by stepping on the foot pedal during die change.   b. Saw 482 was not unplugged to prevent accidental activation when changing saw blades  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET)

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  On or about April 4, 2017, employees were exposed to struck-by and amputation hazards when the following equipment was not guarded or was not adequately guarded:   a. cut-to-length die used on Greenerd 7-ton press 573.  Employees were exposed to amputation hazards when trying parts from the machine.   b. punch die used on Greenerd 7-ton press 439.  The sides and back of the guard were open to permit employees to push slugs away from the punch with a stick.  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.252(b)(2)(iii): Workers and other persons adjacent to the welding area were not protected from the rays by noncombustible or flameproof screens or shields:  On or about April 12, 2017, employees walking and working in the area were exposed to welding rays when there were no curtains around the manual weld station located near press 573 in the SCD department.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control:      On or about May 24, 2017, an employee was exposed to struck-by and amputation hazards when the employee had not received authorized employee training when performing work such as, but not limited to, changing blades on cord and plug equipment such as, but not limited to, saws.    The was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 987417, citation number 1, item number 2 and was affirmed as a final order on 11/27/2015, with respect to a workplace located at1290 Barrow Industrial Parkway, Winder, GA.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
