29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms: Instances including, but not limited to, the following: a) Job site, 8000 McKnight Road, Pittsburgh, PA: On or about April 13, 2017, during construction activities, the employer did not ensure that guardrail systems were installed along all open sides and ends of platforms in that along the 5th frame of scaffolding, no midrails were in use, leaving an approximate 3 foot tall opening along the sides of the scaffolding, exposing an employee to an approximate 32 foot fall hazard. b) Job site, 8000 McKnight Road, Pittsburgh, PA: On or about April 13, 2017, during construction activities, the employer did not ensure that guardrail systems were installed along all open sides and ends of platforms in that along the 5th frame of scaffolding, there was an opening in an end frame, determined to be approximately 30 inches tall by 18 inches wide, exposing an employee to an approximate 32 foot fall hazard. c) Job site, 8000 McKnight Road, Pittsburgh, PA: On or about April 13, 2017, during construction activities, the employer did not ensure that guardrail systems were installed along all open sides and ends of platforms in that along the 5th frame of scaffolding, where the scaffold frames changed direction, there were no guardrails installed. A section of cross bracing was placed at an angle from one scaffold section to another, leaving a space of approximately 2 feet by 4 feet open, exposing an employee to an approximate 32 foot fall hazard.

29 CFR 1926.451(g)(4)(ii): The top edge height of toprails or equivalent member on supported scaffolds manufactured or placed in service after January 1, 2000 were not installed between 38 inches (0.97 m) and 45 inches (1.2 m) above the platform surface: Instances including, but not limited to, the following: a) Job site, 8000 McKnight Road, Pittsburgh, PA: On or about April 12, 2017, during construction activities, the employer did not utilize appropriate fall protection systems/devices in that the scaffold system in use had no toprails installed in all places along the top of the 4th scaffold frame section and existing cross bracing in these areas were determined to be approximately 53 inches above the platform, exposing an employee to a fall hazard of up to approximately 26 feet.
