On or about April 13, 2017, employees operated core making machines that involved silica sand and large amounts of sand and core debris were allowed to accumulate on passageways and equipment and were not kept clean.

29 CFR 1910.303(g)(1)(i)(A): For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening whwere they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:    On or about April 13, 2017 employees accessed electrical panels in the shop where the provided depth of working space was less than indicated in Table S-1:    a) In the front area of the main shop the depth of the working spaces for several panels up to 480volts were obstructed by carts of equipment, extra supplies and core boxes that made the depth of working space less than the 3.0ft contained in Table S-1.    b) In the main shop near coremaking machine #6, electrical panels boxes were obstructed by a table of equipment and extra supplies that made the depth of working space less than the 3.0ft contained in Table S-1.    c) In the back locker/break room of the shop, electrical panel boxes were obstructed by refrigerators and garbage cans that made the depth of working space less than the 3.0ft contained in Table S-1.
