29 CFR 1926.502(d)(21): Personal fall arrest systems were not inspected prior to each use for wear, damage, and other deterioration, and/or defective components were not removed from service: 1. Roof, 117 Spring Garden Street, Philadelphia, PA 19123-3006 - The employer did not inspected the rope that was part of a Guardian personal fall arrest system prior to each use for damage and deterioration. Also, the defective component was not removed from service. Employees were exposed to the hazard of falling from the roof that was five (5) stories above the ground. Observed On or About 02/22/17
