29 CFR 1926.404(b)(1)(i): Employer did not use either ground fault circuit interrupters as specified in paragraph (b)(1)(ii) of this section, or an assured equipment grounding conductor program as specified in paragraph (b)(1)(iii) of this section to protect employees on construction sites. a) Naval Weapons Station, Building 479: On or about 04/14/2017, the 50 ft. red extension cord used for temporary power to energize a three-way GFCI pigtail, printer, radio, laptop computer and cell phone was not protected with a ground fault circuit interrupter device or an assured equipment grounding conductor program. The ground fault circuit interrupter used was not plugged directly into the permanent power of the building. No abatement certification or documentation required.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: a.) Naval Weapons Station, Building 479: On or about 04/14/2017, the 50 ft. red extension cord used for temporary power to energize a three-way GFCI pigtail, printer, radio, laptop computer and cell phone, conductor wires at the male end were pulled away from the attachment plug thus caused strain on the internal terminals. No abatement certification or documentation required.

29 CFR 1926.150(c)(1)(viii): Portable fire extinguishers were not inspected periodically in accordance with maintenance and use of portable fire extinguishers, N.F.P.A. No. 10A-1970: a) Naval Weapons Station, Building 479: On or about 04/14/2017, three(3) ABC type fire extinguishers were not inspected periodically and maintained in accordance with maintenance and use of portable fire extinguishers, NFPA No.10A-1970. No abatement certification or documentation required. Note: Periodic inspection of fire extinguishers shall include a check of at least the following items and documented: a) Location in designated place. b) No obstruction to access or visibility. c) Operating instructions on nameplate legible and facing outward. d) Safety seals and tamper indicators not broken or missing. e) Pressure gauge reading or indicator in the operable range or position. f) At least monthly, the date the inspection was performed and the initials of the person performing the inspection shall be recorded. g) Records shall be kept on a tag or label attached to the fire extinguisher, on an inspection checklist maintained on file, or in an electronic system (e.g., bar coding) that provides a permanent record.
