29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.   a. On or about April 14, 2017, employees engaged in construction activities at the above referenced worksite were working in areas requiring personal protective equipment to safeguard against falling, flying debris, and overhead hazards associated with aerial lift operations. Employees were thereby exposed to struck-by hazards likely to cause head injuries.     There is no abatement certification of documentation required for this item.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift.    a.  On or about April 14, 2017 at the above addressed jobsite, an employee working in an aerial lift was not properly attached to the boom or basket by the use of a lanyard, thereby exposing the employee to the hazards associated with a fall of more than fifteen (15) feet.   There is no abatement certification of documentation required for this item.

29 CFR 1926.454(a): The employer shall have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. The training shall include the following areas, as applicable:   a.  On or about April 14, 2017 at the above addressed jobsite, an employee using an aerial lift to perform siding operation on the outside of the building was not authorized and or trained to recognize the hazards associated with the use of aerial lifts, thereby exposing the employee to the hazards associated with falls.    Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
