29 CFR 1910.132(a):  Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:    1) On or about April 13, 2017, and at prior times, employees were exposed to hazards when cleaning the walls of the car wash bay with chemicals that contain sulfuric acid, phosphoric acid, and sodium hydroxide.  The employer did not provide and require employees to use chemical protective clothing such as aprons or coveralls, and chemical protective gloves.    2) On or about April 13, 2017, and at prior times, employees were exposed to hazards when exchanging containers of Prime Pak HPH and Lava Foam in the mechanical room that contain potassium hydroxide, phosphoric acid, and sulfuric acid.   The employer did not provide and require employees to use chemical protective gloves.

29 CFR 1910.133(a)(1): 3 29 CFR  1910.133(a)(1): Protective eye and face equipment was not required where there was a reasonable probability of injury that could be prevented by such equipment:    1) On or about April 13, 2017, and at prior times, the employer did not provide chemical goggles or face shields when employees were exposed to eye and face hazards while working with acids and corrosives at the following locations:  a) sulfuric acid, phosphoric acid, and sodium hydroxide when cleaning the walls of the car wash bay;  b) potassium hydroxide, phosphoric acid, and sulfuric acid when exchanging containers of Prime Pak HPH and Lava Foam in the mechanical room.

29 CFR 1910.1200(h)(1): The employer did not provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area:    1) On April 13, 2017, and at prior times, employees worked with highly corrosive and caustic chemicals such as  sulfuric acid, phosphoric acid, sodium hydroxide, and potassium hydroxide.  The employer did not provide employees with effective information and training on the hazards of the chemicals in the work area, including the location of safety data sheets, the hazards of the chemicals, and measures employees can take, such as work practices and personal protective equipment, to protect themselves from the hazards.
