29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Meadow Glen at Skippack, Lot 259 - on or about April 12, 2017, employees were performing frame carpentry work next to an improperly guarded floor opening, approximately 8 feet, 6 inches below the upper level. b) Meadow Glen at Skippack, Lot 259 - on or about April 12, 2017, employees were performing frame carpentry work, approximately 9 feet, 3 inches above grade, near an improperly guarded patio door wall opening. c) Meadow Glen at Skippack, Lot 260 - on or about April 12, 2017, employees were performing frame carpentry work while obtaining material from a storage location located next to the basement stairwell opening, approximately 8 feet, 8 inches above the lower level, without having a secured floor hole cover marked HOLE or COVER. d) Meadow Glen at Skippack, Lot 260 - on or about April 12, 2017, employees were performing frame carpentry work while obtaining material from a storage location located next to an improperly guarded patio door wall opening 10 feet, 8 inches above grade.

29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: a) Meadow Glen at Skippack, Lot 259 - on or about April 12, 2017, employees were performing frame carpentry work while accessing the first floor of the structure, approximately 32 inches above grade, without being provided with an appropriate means of access such as, but not limited to, a ramp or stairway, to reach the upper level. b) Meadow Glen at Skippack, Lot 260 - on or about April 12, 2017, employees were performing frame carpentry work while accessing the first floor of the structure, approximately 32 inches above grade, without being provided with an appropriate means of access such as, but not limited to, a ramp or stairway, to reach the upper level.
