29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    Location: 1604     On or about: 12 Apr 2017, 11 Apr 2017, and 10 Apr 2017    a) Employees worked on the 2nd floor stage area exposed to falls of up to 18' and were not trained to be able to recognize, minimize or eliminate the fall hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    Location: 1604 Broadway Ave        On about 04/12/17    a) Employees were cutting the deck and steel beams of the second floor with an Oxygen & Acetylene torch on the 2nd floor of a construction rehabilitation project without using a fall protection system.  An employee fell from the 2nd floor stage area to the first floor.     On or about 04/11/17    b) Employees were removing fireproofing material, transitioning with equipment and removing debris on the second floor of a construction rehabilitation project without using a fall protection system.    On or about 04/10/17    c) Employees were setting up torch equipment and lowering beams with rope to the lower floor without using a fall protection system.
