On or about 04/11/17    b) Employees were removing fireproofing material, transitioning with equipment and removing debris on the second floor of a construction rehabilitation project without using a fall protection system.    On or about 04/10/17    c) Employees were setting up torch equipment and lowering beams with rope to the lower floor without using a fall protection system.
