29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. Location: 1604 Broadway Ave On about 04/12/17 a) Employees were cutting the deck and steel beams of the second floor with an Oxygen & Acetylene torch on the 2nd floor of a construction rehabilitation project without using a fall protection system. An employee fell from the 2nd floor stage area to the first floor. On or about 04/11/17 b) Employees were removing fireproofing material, transitioning with equipment and removing debris on the second floor of a construction rehabilitation project without using a fall protection system. On or about 04/10/17 c) Employees were setting up torch equipment and lowering beams with rope to the lower floor without using a fall protection system.
