29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a. On or about April 14, 2017, at the back of the building, hard hats were not worn by a subcontractor employee on a wooden platform elevated by a forklift and a subcontractor employee on a supported scaffold platform. Subcontractor employees were exposed to struck-by hazards when erecting scaffolding.

29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms without being specifically designed by the manufacturer for such use: a. On or about April 14, 2017, at the back of the building, two subcontractor employees were on a wooden platform that was elevated by a forklift. The wooden platform was not approved for lifting personnel with the forklift and subcontractor employees were exposed to fall and struck by hazards.

On or about April 14, 2017, at the back of the building, two subcontractor employees were on a wooden platform that was elevated by the forks of a forklift when the forklift traveled horizontally and were exposed to fall and struck by hazards.
