29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: a. On or about April 14, 2017, at the back of the building, hard hats were not worn by an employee on a wooden platform elevated by a forklift and an employee on a supported scaffold platform. Employees were exposed to struck-by hazards when erecting scaffolding.

On or about April 14, 2017, at the back of the building, two employees were on a wooden platform that was elevated by a forklift. The wooden platform was not approved for lifting personnel with the forklift and employees were exposed to fall and struck by hazards.
