29 CFR 1926.501(b)(10): The employer did not designate a competent person to monitor the safety of other employees and/or did not ensure that the safety monitor complied with the requirements listed in (h)(1)(i) through (h)(1)(v) of this section: a) On or about March 14, 2017, and at times prior, Mountain Construction & Management Co. Denver Division, LLC dba Roof Worx, LLC, as the controlling employer, did not ensure that employees of Big O Roofs, LLC (subcontractor) were protected from fall hazards in that, a roof monitor was not assigned when workers were outside of a warning line on a flat roof.
