29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:     For the employee required to work outside the area of the frame scaffold with fall protection provided by guardrails and the wall of the structure.  The employee wearing a harness and lanyard on or about April 12, 2017 and times prior there to, were not tied off to a structural member of the fabricated scaffold framing.  This exposes the employee to the hazard of falling.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of platforms before being released for use by employees other than erection/dismantling crews:     For the employees laying stone on the Sixth (6) tier of the self-supporting scaffold on or about April 12, 2017 and times prior there to,  The employer did not install a guardrail system on the north end of the working level.  This exposes employees to the hazard of falling.

29 CFR 1926.452(c)(4): End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means in areas where uplift could occur:     For the employees working on the scaffold on or about April 12, 2017 and times prior there to, the employer did not pin the scaffold frames together when operating an all terrain material handler, delivering mortar and stone onto the scaffold to prevent uplift and/or collapse.  This exposes employees to the hazard of falling.
