On or about April 8, 2017, 3rd floor back balcony, two employees removing pylons were not protected from falling 22 feet 8-inches to the ground by the use of a fall protection system. b. On or about April 8, 2017, 2nd floor front balcony, two employees resetting pylons were not protected from falling 12 feet to the ground by the use of a fall protection system.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA: a. On or about April 8, 2017, the employer failed to inform OSHA within 24 hours after the hospitalization of one of their employees from a work related accident. The employer was advised of the Victim's accident at 11:15 am EST on April 8, 2017 and was advised of the hospitalization on April 8, 2017 around 4:00 pm EST.
