29 CFR 1910.23(b)(10): (10) Any ladder with structural or other defects is immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with �1910.145 and removed from service until repaired in accordance with �1910.22(d), or replaced; a. At the workplace in the Receiving Area, a Tri-Arc Manufacturing Co. steel rolling ladder had approximately four inch wide holes in the ladder's bottom two treads, exposing employees to trip and fall hazards while ascending and descending. Employees were using the ladder to access merchandise stocked on the upper shelves.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc. a. At the workplace in the Receiving Area, machine guarding was not provided to protect the operator and other employees from hazards created by flying debris and access to moving parts of the Philadelphia Tramrail baler/compactor. The baler/compactor's limit switch and gate latch had been bypassed.

At the workplace in the Receiving Area, employees were exposed to a pedestal fan that did not have a grounding pin. The grounding pin had been broken off from the fan's plug.
