29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions. An inspection was not conducted by the competent person prior to the start of work and as needed throughout the shift: (a) on the jobsite, near unit 3211 - an excavation, the surrounding area, and the bracing support system was not inspected by a competent person when a trench excavation, 8in wide by 3ft deep, was opened adjacent to and 20 inches below the level of the footing of an electrical panel wall (11ft,1in wide by 7ft,5in high by 8.5in thick) for the purpose of running a conduit from the transformer to the electrical panel and meters to be mounted on the wall, exposing employees to the hazards of being struck-by and/or crushed by the wall in the event of it falling over; on or about April 10, 2017.
