29 CFR 1926.501(b)(13): "Residential construction." Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system. On 4/4/2017, employees from Zun Roofing LLC were working on the roof of a residential building located at 122 Langtree Drive in Wentzville, MO. The Owner and four employees were engaged in roofing work approximately 10 feet above a lower level. Exposed employees were not utilizing guardrail systems, a safety net system, or a personal fall arrest system. The employees were observed on the roof of the building without fall protection, exposing themselves to a fall to the ground below.

29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access. On 4/4/2017, workers from Zun Roofing LLC, repairing a roof at a residential building located at 122 Langtree Drive, Wentzville, MO were using a ladder improperly. The ladder was not extended three feet beyond the landing or edge of the roof.
