29 CFR 1910.119(e)(1): The process hazard analysis was not appropriate to the complexity of the process and did not identify, evaluate, and control the hazards involved in the process: (a) Xcel Energy, at 6211 Weld County Rod 51, Keenesburg, CO 80643: On or before April 6, 2017, the Rocky Mountain Energy Center (RMEC)'s process hazard analysis (PHA) for the use of anhydrous ammonia (NH3) was not appropriate to the process, and did not identify, evaluate, and control the hazards in the process. The employer's PHA did not include an evaluation of NH3 travelling under pressure from the Chromalox Industrial U tube screw plug immersion heaters through the heater conduit to the service panel. When an employee opened the service panel, he was exposed to NH3. This condition could result in exposure to respiratory hazards from the NH3.

29 CFR 1910.119(f)(1)(i)(B): The employer did not develop and implement written operating procedures that provide clear instructions for safely conducting activities involved in each covered process consistent with the process safety information and did not address normal operations: (a) Xcel Energy, at 6211 Weld County Road 51, Keenesburg, CO 80643: On and before April 6, 2017, the employer did not develop and implement written operating procedures that provided clear instructions for safely operating the Chromalox Industrial U tube screw plug immersion heaters. The employer installed 4 Chromalox Industrial U tube screw plug immersion heaters for each of two anhydrous ammonia (NH3) tanks. The written operating procedures did not address the connections and the control systems needed to ensure that adequate levels of NH3 were maintained in the heaters to safely operate the heaters in normal operations. The NH3 travelled under pressure from the heaters through the heater conduit to the service panel. When an employee opened the service panel, he was exposed to NH3. This condition could result in exposure to respiratory hazards from the NH3.

29 CFR 1910.307(c)(2): Equipment, wiring methods, and installations of equipment in hazardous (classified) locations were not safe for the hazardous (classified location), nor approved for the hazardous (classified) location or the ignitable or combustible properties of the specific gas, vapor, dust or fiber that was present: (a) Xcel Energy, at 6211 Weld County Road 51, Keenesburg, CO 8043: On and before April 6, 2017, the employer did not ensure that the equipment, wiring methods, and installations of equipment in hazardous (classified) locations were safe for the ignitable or combustible properties of the anhydrous ammonia (NH3) that was present. The employer did not ensure that the installation of the Chromalox Industrial U tube screw plug immersion heaters included protection from the accumulation of NH3 vapors travelling from the heaters through the heater conduit to the heater service panel. The employer connected the heaters to the service panel with 3/4 inch metal conduit. The employer installed an atmospheric seal in the conduit that was not rated for conditions under pressure. The seal failed, and NH3 vapors travelled from the heaters through the conduit to the service panel. When an employee opened the service panel, he was exposed to NH3. This condition could result in exposure to respiratory hazards from the NH3.
