29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: On April 18, 2017, and at times prior to, annual audits to evaluate the effectiveness of the company's machine specific energy control procedures were not completed as required. A review of company's energy control procedures revealed that the procedures were deficient in that the verification step was missing. Energy sources used at the facility include; but are not limited to, electrical, hydraulic, pneumatic and thermal energy.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about April 18, 2017, employees who performed service and maintenance activities on or around the company's hydraulic presses were not required to: 1. Prior to the start of any work activity on machinery and/or equipment that had been lock out or tagged out, verify that isolation and deenergization of the machine or equipment had been accomplished. Although the employer developed and enforced the use of energy control procedures, they were deficient in that, verification of isolation [(d)(6)] steps were not included or applied to protect employees from machine servicing hazards from unintended start-ups and energizing of the powered presses.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle. a. On or about April 18, 2017, the employer did not ensure that the point of operation was effectively guarded for the Greenlee 35 ton hydraulic trim press.

On April 18, 2017, and at times prior to, the employer did not inspect and maintain the safe condition of a point of operation protection device (pull back handles). The pull back handles (which were used in lieu of a two-hand control unit) are used as a safeguard device to ensure that employees maintain concurrent hand placement during the cycling of the Greenlee 35 ton, hydraulic trim press.
