29 CFR 1926.95(a): Personal protective equipment, including that for eyes, head and extremities, were not used when such equipment was necessary by reason of hazards from processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment of any part of the body through absorption, inhalation or physical contact: a) 2712 Hylan Blvd., Staten Island, NY 10306. Employees are exposed to crushing hazards during demolition work. while removing a framed window, weighing approximately 175 pounds, the window fell and landed on the employees right foot. The employee was not wearing any foot protection, i.e. steel toe boots. Occurred on 04/18/17
