29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. At a commercial building located at 13902 Century Lane, Grandview, Missouri: An employee was exposed to fall hazards. The employer did not provide or require the use of fall protection, covers, or guardrail systems erected around skylights while conducting roofing work. On or about March 21, 2017, an employee was injured when working on the roof stepped through a skylight and fell approximately 19 feet landing on a concrete floor.

Brothers Home Improvements or B.H.I. did not provide fall hazard training to an employee who was exposed to fall hazards. Fall hazard training was not given to recognize fall hazards and use of fall protection.
