29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level was protected from falling by a guardrail system, safety net system, travel restraint system or personal fall arrest system. (a) On or about March 13, 2017, and at times prior, employees were exposed to falls of 9 feet from the mezzanine where materials are loaded by the forklift, to the warehouse floor below. The area was unguarded. An employee fell and suffered sever injuries. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
