On or about April 13, 2017 at 89-02 37th Avenue, Jackson Heights, NY 11372    An emergency exit door was observed blocked by a metal hand truck preventing prompt and safe egress from the building in the event of a fire and/or other emergency.    B.) On or about April 13, 2017 at 89-02 37th Avenue, Jackson Heights, NY 11372    An emergency exit route leading to a 1st floor exit door was observed obstructed by a permanently installed conveyor belt preventing prompt and safe egress from the building in the event of a fire and/or other emergency.    C.) On or about September 1. 2017 at 89-02 37th Avenue, Jackson Heights, NY 11372    An emergency exit route leading to a 1st floor exit door was observed obstructed by a hand truck containing cartons of canned goods preventing prompt and safe egress from the building in the event of a fire and/or other emergency. On or about April 13, 2017 at 89-02 37th Avenue, Jackson Heights, NY 11372 (Meat Department)    The employer failed to provide employees with eye protection who are required to use hazardous chemicals such as but not limited to;  E-Z Suds containing Isopropyl Alcohol, Epic Dical 2 containing Quaternary Ammonium Compounds and Clorox Bleach containing Sodium Hypochlorite to clean equipment throughout the building.    B.)
