29 CFR 1910.219(c)(4)(i): Unguarded projecting shaft end(s) did not present a smooth edge and end and projected more than one half the diameter of the shaft: a) On or about 04/20/17 in the Model B room; employees were exposed to being caught on the 4 inch unprotected rotating (one inch diameter) shaft and flanged clevis end of the high speed chip auger on the Davenport screw machine # 17. b) On or about 04/20/17 in the Model B room; employees were exposed to being caught on the 5 inch unprotected rotating (one inch diameter) shaft and flanged clevis end of the high speed chip auger on the Davenport screw machine #24. c) On or about 04/20/17 in the Air Feed room; employees were exposed to being caught on the 4.5 inch unprotected (one inch diameter) rotating shaft and flanged clevis end of the high speed chip auger on the Davenport screw machine #30. d) On or about 04/20/17 in the Model B room; employees were exposed to being caught on the 4 inch unprotected (one inch diameter) rotating shaft and flanged clevis end of the high speed chip auger on the Davenport screw machine #27. e) On or about 04/20/17 in the Model B room; employees were exposed to being caught on the 3 inch unprotected (one inch diameter) rotating shaft and flanged clevis end of the high speed chip auger on the Davenport screw machine #11.
