29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a): Maintenance Shop: On or about February 1, 2017, the employer did not ensure that machine guarding on the Wilton drill press was properly adjusted to protect employees from the rotating chuck. (b): Maintenance Shop: On or about April 18, 2017 and times prior, the employer did not ensure that machine guarding was provided on an Enco 7" bandsaw (Model 137-3178) to protect employees from the unused portion of the blade.

29 CFR 1910.304(g)(5): The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective: (a) Plate Room: On or about April 19, 2017, an employee would weigh rolls of material using a Brechbuhler Scales, Inc. scale (Model 350; Serial No. 975016) which had a plug that was missing the ground pin that would allow for a continuous, effective path to ground.
