29 CFR 1926.652(a)(1): Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section. a. Located at the Jewett Water Tank project in Jewett, OH, cave in protection was not provided while an employee was laying pipe in a trench that was approximately 7 feet deep, exposing the employee to a cave in hazard.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. The employer did not report an in-patient hospitalization of an employee within 24 hours of receiving knowledge of the hospitalization. On or about March 28, 2017 an employee was hospitalized after being struck by a excavator bucket, sustaining a back injury.
