29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:      a) Saw Area - On or about April 20, 2017, the employer failed to establish a program that included energy control procedures for employees to follow, employee training, and periodic inspections to protect employees from injury caused by the unexpected energization, start up or release of stored energy when performing set up and servicing/maintenance of equipment, including horizontal band saws, trim saws, and chop saws.

29 CFR 1910.178(p)(1): Powered industrial truck found to be in need of repair, defective, or in any way unsafe, were not taken out of service until it has been restored to safe operating condition:       a) Shop Area - On April 20, 2017, the employer did not ensure that the Clark Forklift, Model #GCX 25, Serial #G4 230 06 92 7960, which had broken front light covers, and the Toyota Forklift, Model #8FGU30, Serial #35651, which was missing two rubber foot pedal covers, were removed from service until repairs could be made.
