29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about April 20, 2017, employees operating the GMM intra 36 CNC, s/n 4107, were exposed to hazards associated with the point of operation while cutting countertops because the interlock on the safety gate was not functional and allowed the saw to be operated without the safety gate and interlock in place.

29 CFR 1910.179(j)(4)(iii): Standby crane(s) were not inspected at least semi-annually in accordance with the requirements of 1910.179(j)(2) and 1910.179(m)(2) On or about April 20, 2017, employees operated the 1-ton Handling Systems Intl jib crane on an infrequent basis to move countertop slabs and other heavy material and the crane had not been inspected since it was initially installed in 2014.
