29 CFR 1910.305(b)(2)(i): All pull boxes, junction boxes, and fittings shall be provided with covers identified for the purpose. If metal covers are used, they shall be grounded. In completed installations, each outlet box shall have a cover, faceplate, or fixture canopy. Covers of outlet boxes having holes through which flexible cord pendants pass shall be provided with bushings designed for the purpose or shall have smooth, well-rounded surfaces on which the cords may bear.  a) In the basement food prep room, an electrical box did not have a cover properly attached exposing live insulated wires. The cover had been unscrewed except for one screw and had been moved to the side so that it no longer covered the wires inside the box exposing employees to an electrical shock hazard.    b) In the mechanical room an electrical box did not have a cover properly attached exposing live insulated wires. The cover had been unscrewed except for one screw and had been moved so that it only partially covered the wires inside the box.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables shall be connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws.  a) In the mechanical room an electrical box was observed to have the metal conduit pulled out of the boxes wire opening exposing live insulated wires. The electrical wires were not protected by strain relief placing strain directly on the wire connectors and exposing employee to an electrical shock hazard.

29 CFR 1910.305(g)(1)(iv): Unless specifically permitted otherwise in paragraph (g)(1)(ii) of this section, flexible cords and cables may not be used:    29 CFR 1910.305(g)(1)(iv)(A): As a substitute for the fixed wiring of a structure;    a) In the mechanical room, an orange extension cord used to supply electrical power to the soda pumps was being used as a substitute for fixed wiring exposing employees to an electrical shock hazard.

29 CFR 1910.22(a)(3): (3) Walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice.    a) At the facility the floor of the food preparation room had broken tile around the floor drain. The depression caused by the missing tile created a tripping hazard and when filled with liquid created a slip hazard.

29 CFR 1910.1200(h)(1): Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new physical or health hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and material safety data sheets.  a) At the restaurant, where employees used chemicals such as, but not limited to Orkin Actizyme Concentrate, Array Germicidal Bleach & Disinfectant, ChemStation 7086, and Avance Grease Cutter, employees had not received information and training on the presence of the chemicals, the health hazards of the chemicals and the means the employer had provided to protect them from those hazards.
