29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    On or about April 20, 2017, at the Mill 1 OD Scarf chopping area, where an employee was exposed to a struck-by hazard when performing a tool change without utilizing the written energy control procedure for the control of potentially hazardous energy.      BOOMERANG TUBE, LLC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD 29 CFR 1910.147(C)(4)(I), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 983919 CITATION NUMBER 3 ITEM NUMBER 1 AND WAS AFFIRMED AS A FINAL ORDER ON 08/24/2015, WITH RESPECT TO A WORKPLACE LOCATED AT 1100 FM 3661 ROAD, LIBERTY, TX 77575.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.      On or about April 20, 2017, in the Mill 2 flying cut off area, at the runout table, where employees were exposed to struck-by and caught-between hazards when working in close proximity to moving pipes without a guard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.   On or about April 20, 2017, in the slitter recoil area, where an employee was exposed to a caught-in and struck-by hazard when banding recoiled steel without a safeguard against the struck-by and caught-in hazard when circumventing the barrier guard.
