29 CFR 1910.217(c)(3)(iii)(c): Presence sensing point of operation device(s) on mechanical power press(es) were not constructed so that a failure within the system would not prevent the normal stopping action from being applied to the press when required: On or about March 28, 2017, in the Accessories department, the presence sensing device on the Verson part revolution mechanical press brake #1182, S/N 22174-20-6-65, used to form aluminum parts allegedly did not prevent the machine from cycling when the operator's hand was inadvertently placed into the point of operation. This alleged sensor exposed the operator to hazards such as,l but not limited to, fractures and amputations.

29 CFR 1910.217(e)(1)(i)(A): Periodic and regular inspections of each power press were not conducted to ensure that all of its parts, auxiliary equipment, and safeguards, including clutch/brake mechanism, antirepeat feature, and single-stroke mechanism, are in safe operating condition and adjustment: The employer did not conduct thorough inspections of the Verson part revolution mechanical press brake #1182, s/n 22174-20-6-65, in the Accessories department to ensure all parts and mechanisms such as but not limited to the presence sensing device was in safe working condition. On or about March 28, 2017, the light curtain installed on the press did not prevent the machine from cycling when the operator's hand was inadvertently placed into the point of operation while forming aluminum parts. Employee was exposed to hazards such as but not limited to fractures and amputations.
