This most recently occurred on April 20, 2017 at the facility located at 11002 Sapp Brothers Dr, Omaha, NE 68138. The employer has employees working on and maintaining 963C trackloaders without having developed and implemented written procedures that specifically identify the steps necessary to control the release of stored energy.
