29 CFR 1910.179(j)(3): Complete inspections of the crane was not being performed at intervals as generally defined in paragraph (j)(1)(ii)(b) of this section:      On or about April 20, 2017, and at times prior thereto, at this Concrete Products Manufacturing shop production area, where employees hoisted heavy equipment and concrete molds with overhead cranes, the employer did not perform any complete periodic inspections for the past 12 months for any defects of his overhead cranes components or functional operating mechanisms, such as:    a) Overhead crane, left side of the shop (Coffing JLC, 2 Ton); and,  b) Overhead crane, right side of the shop (Coffing JLC, 2 Ton).

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:       On or about April 20, 2017, and times prior thereto, at this establishment, employees used improper wiring to energized the equipment in a lieu of the original a manufacture power cord, such as, but not limited to:    a) Romex wire to energized the concrete mixer;  b) Extension cord that energized the left side overhead crane (Coffing JLC, 2 Ton);  c) Extension cord that energized the right side overhead crane (Coffing JLC, 2 Ton);and,  d) Extension cord that energized the Welding machine (P&H Harnimatic, Model: RF-3).

A cord connected to a concrete mixer, and, b. A cord connected to the left side overhead crane (Coffing JLC, 2 Ton); c. A cord connected to the right side overhead crane (Coffing JLC, 2 Ton); d. A cord connected to the Welding machine (P&H Harnimatic, Model: RF-3); e. A cord connected to a commercial fan; and, f. A cord connected to a mold concrete shaker/vibrator.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  On or about April 20, 2017, and times prior thereto, at this Concrete Products Manufacturing shop, employees involved in fabricating concrete products, mixing cement, sand or when those molds are sprayed, lubricated and cleaned with hazardous chemicals such as, but not limited to, corrosives Portland Cement, manufactured concrete sand, gravel and stones, Lacquers, Thinners, Paints and Clear Coats.  The employer had not developed a program which would address labeling and other forms of warning on chemicals containers, Safety Data Sheets and employees information and training on the hazards associated with chemicals used at this site.  The written program must also contain the following:  a. a list of all hazardous chemicals on site; b. the methods the employer will use to inform employees of the hazards associated with non-routine tasks involving       chemicals, such as a spill; c. the hazards of chemicals contained in piping that is not labeled; and, d. the method the employer will use to inform other employers (contractors) of the chemicals their employees might        be exposed to while performing duties at this site.
