On or about 4/20/2017 in the G-Dock facility at 4300 Plesantdale Road Atlanta, GA: Several of the emergency exit lights over the exit doors in the G-Dock and facility areas were not operating properly to provide the employees lighted access to exit the building safely.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.    On or about 4/20/2017 in the G-Dock facility at 4300 Plesantdale Road Atlanta, GA: Where employees operated powered industrial trucks daily in the facility, they were only provided an uninstructed video to watch and written test as training and evaluation.  The employer had not conducted the practical training and the workplace evaluation of the employees as required under the applicable standard.

29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition(s):     On or about 4/20/2017 in the G-Dock facility at 4300 Plesantdale Road Atlanta, GA: Where employees operated powered industrial trucks with damaged exposed electrical wiring, hydraulic pulleys', unserviceable tires, and missing fork locking pins. The employer failed to remove the powered industrial trucks from service until the repairs could be made.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falling racks and falling product when racks that were damaged and racks with job-made repairs were used to store materials.  This hazard was recognized by the employer who had performed job-made repairs on some of the racks.      On or about 4/20/2017 in the G-Dock facility freezers at 4300 Plesantdale Road Atlanta, GA: Employees were exposed to struck-by hazards while performing work around damaged racks. The racks were in use with materials stored in all locations of the racks.  The materials stored weighed 250-300 pounds per stack.  Forklifts are used to access the materials in the racks in freezers 22 through 26 in the facility.  Racks observed with defects and deficiencies such as but not limited to:   a.     Twisted support legs and bent braces  b.     Broken metal welds on the support legs and cross braces  c.     Improper job-made welding repairs made by the employer      AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD TO CORRECT THESE HAZARDS IS:  To follow American National Standard Institute (ANSI) MH16.1:2012, Specification for the Design, Testing, and Utilization of Industrial Storage Racks, Section 1.4.9, which states, "Upon any visible damage, the pertinent portions of the rack shall be immediately isolated by the user until the damaged portion is evaluated by a storage rack design professional.  Before allowing the rack to be placed back into service the design professional must certify that the rack system and/or the repaired components have been restored to at least their original design capacity.
