29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:    (a) On or about 4/20/2017 - 5547 Bridle Way, Cedar Creek Lot #26, Bessemer, AL, employees were exposed to fall hazards of approximately 10 feet above ground level while carrying a pneumatic air gun and securing the underlay on a new single family residence.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) On or about 4/20/2017 - 5547 Bridle Way, Cedar Creek Lot #26, Bessemer, AL, the employer did not train each employee to recognize the hazards of falling and how to minimize these hazards when working at heights of approximately 10-feet above the ground to install the underlay and shingles on a roof.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder:    (a) On or about 4/20/17 - at 5547 Bridle Way, Cedar Creek Lot #26, Bessemer, AL, a 24-foot aluminum ladder used to access the roof was not extended at least 3-feet above the landing surface, was not secured to a rigid support, and/or a grasping device was not provided.
