29 CFR 1910.269(d)(6)(iv): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees: Employees were exposed to injury from an electrical shock hazard. The employer did not ensure lockout devices were affixed to the anti-racking device provided by the manufacturer. The lack of lockout/energy isolation was discovered on the Siemens brand, medium voltage switchgear cubicle.
