29 CFR 1960.8(a): The head of each agency shall furnish to each employee employment and a place of employment which are free from recognized hazards that are causing or are likely to cause death or serious physical harm. 29 CFR 1960.8(a): 29 CFR 1960.8(a): The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm: (a) Spillway: On or about April 20, 2017 and times prior thereto, an employee was not prevented from coming into contact with the rail mounted gantry crane S/N G2JDSC while it was in motion. This resulted in the employee getting in a pinch point that resulted in a severe laceration to his forearm from coming in contact with a moving crane and a stationary ladder.
