29 CFR 1910.134(d)(1)(iii): The employer did not identify and evaluate the respiratory hazard(s) in the workplace; including a reasonable estimate of employee exposures to respiratory hazards and identification of the contaminant's chemical state and physical form: On or about April 21, 2017: Employees working in the production area are exposed to inhalation hazards. The employer did not evaluate the respiratory hazard of the work place to include but not limited to: (a) Employees working in the foam/floatation area, where employees spray a two part foam into the coolers and are exposed to Diphenylmethane Diisocyanate, isomers and homologues (4,4-Methylenediphenyl Diisocyanate). (b) Employees working in the mixing/weighing area are exposed to dust from a medium density powdered plastic polyethylene with UV stabilizers, where the employees transfer the powder to buckets. (c) Employees working in the rotomolding area where the polyethylene is heated and the rotomolders are not vented out of the area.
